PURPOSE To describe results in two cases of a dislocated silicone plate haptic intraocular lens (SPHIOL) managed with vitrectomy and repositioning with scleral suture fixation. METHODS small case series. RESULTS Two patients present with dislocated SPHIOL underwent a 23-gauge vitrectomy with a "handshake" maneuver to pass a 9-0 prolene suture through the positioning holes to allow scleral fixation. Best corrected visual acuity was returned to predislocation level 2 and 8 months after the PPV. CONCLUSIONS SPHIOLs can be managed with scleral suture fixation using this adapted technique, avoiding removal and exchange techniques. IMPORTANCE These are the first reported cases of SPHIOL managed with scleral suture repositioning.
