To the Editors: C lozapine-associated kidney injury, which has been attributed to acute interstitial nephritis (AIN), is a rare but serious adverse drug reaction identified in 13 case reports to date. These cases herald complications that lead to complex clinical scenarios commonly involving presumptive use of antibiotics and combined medical and psychiatric management. One such complication is secondary lithium toxicity, which has been rarely discussed in the 5 reports of lithium use in this setting and illustrated in the presented case.
