These authors report a series of 36 patients with confirmed prostate cancer who underwent 3T magnetic resonance imaging before radical prostatectomy. Wedge-shaped lesions were identified and correlated with whole mount pathology specimens. A total of 35 wedge-shaped lesions were identified, of which 12 were malignant. Ten of the 12 malignant wedge-shaped lesions were markedly dark on apparent diffusion coefficient (ADC) imaging and several had early enhancement on dynamic contrast enhanced imaging. Of the 12 patients with malignant wedge-shaped lesions 1 had Gleason 3 þ 3, 7 had Gleason 3 þ 4, 3 had Gleason 4 þ 3 and 1 had Gleason 4 þ 5 disease. Of the patients with benign wedge-shaped lesions 17 had a discrete histological correlate and 6 did not. The benign lesions consisted of prostatitis, chronic inflammation and nonspecific atrophy. The benign wedge-shaped lesions tended to be mildly hypointense on ADC imaging and did not show early dynamic contrast enhancement. Overall, 7 of 23 benign lesions had marked hypointensity, similar to the malignant lesions. I found this to be an interesting study because we are faced with these wedge-shaped lesions frequently and there has been little study of them. This article alerts us that wedge-shaped lesions may be benign or malignant, and a more careful assessment of ADC values and dynamic contrast enhancement will be helpful in further characterizing them.
