We believe that any effort is extremely important to decrease the number of undiagnosed cases with pressure-overload effect due to hypertension. In this regard, early signs on regional left ventricular (LV) remodeling recently have gained importance. Basal septal hypertrophy (BSH) as the early imaging biomarker can be helpful to diagnose earlier of hypertensive patients. This early sign have been confirmed prospectively by the third-generation microscopic ultrasonography in the small animal model. Predominant involvement of septal wall in disease course in human hypertension was also confirmed using real-time 3 DE, exercise Doppler, and exercise tissue Doppler imaging in this group of patients.
