These effects include the reduction of arterial stiffness and an improvement in the endothelial function,6 an acceleration of myocardial relaxation through the stimulation of sarcoplasmic reticulum calcium-adenosine triphosphatase,7 and the enhancement of myocardial compliance through the reduction of myocardial fibrosis.8 To the best of our knowledge, this is the first report on the use of ivabradine in the treatment of refractory symptomatic ST in a patient with HOCM. In conclusion, the case highlights the therapeutic potential of ivabradine in HOCM complicated by symptomatic ST in patients who are intolerant or resistant to beta blockers and calcium channel blockers.
