To the Editor: P ancreatic pseudocysts (PPs) commonly occur after acute pancreatitis, with complications including rupture, obstruction, invasion into organs, and infections. Spontaneous rupture of a pseudocyst is uncommon, and the management of intraperitoneal rupture of a sterile pseudocyst is unclear. This case report describes the natural history of a sterile intraperitoneal rupture of a PPmanaged conservatively and proposes an algorithm for the management of ruptured pseudocysts.
