the most commonly reported primary being renal cell carcinoma, followed by colorectal, melanoma, breast, squamous cell lung carcinoma, and sarcoma. We hereby report a case of metastatic adenocarcinoma of the lung to the pancreas diagnosed by endoscopic ultrasound (EUS)–guided fine needle biopsy (FNB). When imaging is suspicious or inconclusive, EUS-guided FNB of the pancreas is essential to guide treatment.
