At a time in history when the role of structural factors in shaping the HIV epidemic is not receiving the attention it deserves, the study by The Collaboration of Observational HIV Epidemiological Research Europe (COHERE) [1], published in this issue, provides a social epidemiological analysis of the relationship between educational level and positive and negative outcomes of HIV combination antiretroviral treatment (cART). They perform this analysis on data from several cohorts of people living with HIV in Europe recording educational attainment. We focus on this study because, beyond presenting valuable data, it also provides an opportunity to consider the role of structural factors over time, in a multicausal framework, and the importance of always keeping the big picture in perspective.
