The combination of tenofovir (TFV) disoproxil fumarate and emtricitabine is currently recommended as a first line treatment in HIV care and more recently in preexposure prophylaxis [1]. Headache is one of its frequent adverse effects [2] and is also frequent among HIV patients [3] but, as far as we know, no case of cluster headache has ever been reported yet. Cluster headache includes trigeminal autonomic symptoms such as weeping eye, rhinorrhea, ptosis, myosis, red eye and eyelid edema. We report two original cases of patients who experienced headache strongly evoking cluster headache shortly after the start of TFV–emtricitabine treatment, with positive rechallenge.
