To the Editor: We rejoice in reading that incorporation of alpha-fetoprotein response into hepatocellular carcinoma (HCC) selection criteria [the New York/California (NYCA) score] allows for expansion of Milan criteria. Among the cohort of 1450 patients, 235 patients were originally outside Milan at the time of diagnosis, and 201 (85.5%) would be recategorized into NYCA low (0–2) or acceptable (3–6) risk categories. As most patients with HCC within Milan may be scored below 4 to 6 under NYCA criteria, it would be helpful to know the demographics of those 201 patients, for better identifying patients with HCC outside Milan but potentially would benefit greatly from liver transplantation.
