I with interest the statement of the Declaration of Istanbul Custodian Group, which recommends transparency, traceability, and continuity of care for patients who return home after receiving an organ in another country even if the organ has been received by illegitimate means. They also recommend that transplant organizations should ensure notification of transnational transplant-related activities by health professionals to law enforcement agencies. The World Health Organization estimates that 5% to 10% of organ transplants performed annually takes place as a result of commodification. However, the true extent is not known because efforts to establish an international registry have not been successful. Because this activity is criminal in most countries and attracts heavy punishment, true data are not available. Transplant professionals are hesitant to report patients of suspected commodification-seeking postoperative care because it may trigger an international criminal investigation. I suggest, as an interimmeasure, setting up of international database populated by an anonymous social networking, which may allow recipients and donors to place the characteristics of the hospital and transplant team in the public domain without repercussions to their postoperative care. Anonymous social apps are on the rise, but currently there are 3 major players: Yik Yak, Whisper, and Secret. Whisper claims more than 3.5 billion page views a month, Yik Yak is a regular in the social app download charts, and Secret refuses to reveal exact figures but has received lots of media
