T he shortage of suitable donors for vascularized composite allotransplantations (VCAs) is exacerbated by inadequate tissue preservation methods. Revascularization within several hours of procurement is mandatory, dra-matically reducing the geographical radius for donor recruitment. Ex situ machine perfusion (EMP) has been proven to be a viable method to enhance the allowable time until transplantation, exceeding static cold storage. 1 The next requisite step to advance this technology in the niche of clinical VCA is to optimize the EMP settings as a preparation for the clinical use.
