Although several definitions exist for the syndrome of acute on chronic liver failure (ACLF), it is generally agreed upon that ACLF is a condition of rapid clinical deterioration in a patient with chronic liver disease, associated with multiorgan failure and high short-term mortality.(1) In certain patients who develop ACLF, the only therapeutic option is liver transplantation (LT). However, given the limited supply of donor organs, it is not feasible to transplant all patients with ACLF due to concerns regarding poor post-LT survival, particularly among patients with three or more organ failures, also known as ACLF-3.
