The Organ Procurement and Transplantation Network (OPTN) has recently updated their policies1 related to donors with risk criteria for blood-borne viruses (BBV) and their organ recipients to align with the 2020 U.S. Public Health Service (PHS) guidelines2 . Some of the new policies are anticipated to offer benefits, such as mandating hepatitis B virus vaccination in transplant candidates. Other new policies raise challenges with transplant candidate BBV screening, nomenclature, and the consent process. We would like to highlight some of these issues and discuss potential solutions.
