Prucalopride is a selective 5-hydroxytryptamine type 4 (5-HT4) receptor agonist approved for chronic idiopathic constipation in adults. Here, we report a first lifetime episode of acute mania in a 52-year-old man and hypothesize that recent prucalopride initiation was a precipitating factor.
