DEAR EDITOR, A 51-year-old previously healthy man was observed for a 3-month history of rapidly growing firm nodules in both external auditory canals (EACs) causing hypoacusis (a). Skin biopsy with Congo red stain identified amyloid deposition (b). Subsequent study revealed anaemia, renal insufficiency, monoclonal band of kappa light chains in serum and urine and multiple osteolytic lesions of the ribs. Bone marrow biopsy showed plasmacytic infiltration. In this case, the amyloid light-chain amyloidosis was the first manifestation of multiple myeloma. Its presentation as multiple exophytic nodules occluding the EAC is unique, rather than the diffuse skin thickening of the EAC reported in previous cases.
