We read with interest the recently published paper by Pandeya et al.1 , which assessed the association between aspirin and non-steroidal anti-inflammatory drugs (NSAIDs) and incident keratinocyte cancers (KC) in a cohort study and reported a weak and inconsistent association between aspirin and NSAID use and KC. However, this study design and especially exposure definition, i.e. extent of aspirin and NSAID use, may have led to misclassification of exposure, and thereby to biased results.
