Transplant‐associated thrombotic microangiopathy remains a lethal complication of haematopoietic stem cell transplant and not all patients respond to terminal complement inhibitors. Qi et al. show that hypoxia‐inducible factor‐1α (HIF‐1α) may be a previously unrecognized driver of this endothelial injury syndrome.
