A 70-year-old man with known low-risk prostate cancer on active surveillance received a renal and bladder ultrasound for elevated serum creatinine. The ultrasound showed a right upper pole mass and an echogenic focus in the urinary bladder. A subsequent CT urogram and cystoscopy showed a 5.6 cm solid right upper pole mass, small 1–2 cm bilateral enhancing cortical masses, renal cysts, and bilateral lower lobe pulmonary cysts. Both the cystoscopy and CT urogram were negative for a bladder tumour. The key imaging findings are shown in Fig. 1.
