A 43-year-old man presented with a 2-month history of a progressively worsening widespread itchy rash affecting his trunk and limbs. The rash had started spontaneously without any preceding triggers, particu-larly infective illness, new medications, vaccinations or dental procedure/materials used. He had no relevant past medical or family history, no regular medications and had no known allergies. Physical examination revealed irregularly shaped violaceous papules and plaques and postinflammatory hyperpigmentation on the skin, and ulcerated lesions on oral buccal mucosa. The scalp, nails and genitalia were unaffected.
