Fistula-in-ano (FIA) is one of the most common benign anorectal conditions, yet its treatment is often complex. The key objective of FIA treatment is to achieve a balance between high healing rates and continence preservation. Lay open fistulotomy can result in a success rate of up to 100% but this is at the expense of continence especially in the complex fistula. Recently, several sphincter saving surgical procedures have been advocated for the treatments of FIA with acceptable results; however, the optimal treatment option continues to be debated.
