Anastomotic leak is the most feared complication of ileal pouch anal anastomosis (IPAA). It might require reoperation and also reduce long-term pouch function[1]. Anastomotic leakage has been associated with tension in the mesentery and subsequent ischaemia[2][3]. Lengthening manoeuvres, in order to allow the pouch to reach the pelvis, also might impair the vascularity of the pouch[4]. This article is protected by copyright. All rights reserved.
