Rates of iatrogenic splenic injury during colorectal surgery vary between 1.2% and 8% and splenectomy rates for iatrogenic splenic injury have been reported to be as high as 85% in the setting of colorectal surgery (1). Most often these represent capsular tears due to excessive traction during colonic splenic flexure mobilization (2). Such injuries may be quite challenging to control, despite the fact that laparoscopy may actually be a protective factor against iatrogenic lesions given the better visualization of the spleen and its attachments (3).
