Multiple primary carcinomas (MPC)are defined as more than one synchronous or metachronous cancers in the same individual. The occurrence of adenocarcinoma at two separate locations in the gastrointestinal tract is uncommon, such as in the stomach, small intestine, colon, rectum, etc. Surgery is one of the treatment approaches. We share the case of a 65-year-old man who suffered from upper abdominal pain and distention for two months. During hospitalization, the patient underwent simultaneous totally laparoscopic subtotal gastrectomy and radical anterior resection with extraction of specimens via the anus. The postoperative pathological evaluation of the tumour in the stomach and rectum showed moderately differentiated adenocarcinoma. Both gastric and rectal lymph nodes were negative for disease, (0/33) and (0/12) respectively. Then, the patient received six cycles of SOX regimen as adjuvant chemotherapy and there is no evidence of recurrence until now. Therefore, we believe this technique might be useful as a novel minimally invasive surgery technique for gastrointestinal multiple primary carcinomas.
