Anastomotic leakage (AL) after pouch-surgery is a feared complication. A tension-free anastomosis and adequate perfusion are essential to prevent AL. These requirements may counteract one another. After performing routine ileal lengthening measures involving mobilization of the mesenteric root and peritoneal incisions, vascular ligation of the ileocolic vascular trunk and/or interconnecting terminal ileal branches may be required in case of insufficient pouch-reach (1).
