Splenic flexure mobilization is a key-point of laparoscopic left colectomy and anterior resection. We describe our medio-lateral and caudo-cranial approach. The abdominal cavity is entered and the inferior mesenteric vein (imv), the aorta, the duodenum, the transverse colon and the pancreas are identified. With a gentle traction upward of the imv, we can bluntly dissect the areolar tissue between the Gerota's fascia and the mesentery of the descending colon.
