This video demonstrates the successful management of a 66-year-old woman with a rectovaginal fistula following posterior pelvic exenteration. The patient had rectal adenocarcinoma with uterus invasion. She was submitted to neoadjuvant chemoradiotherapy, followed by laparoscopic total mesorectal excision, associated with a total hysterectomy. A diversion ileostomy was used.
