Cryptococcosis is caused by a yeast-like fungal organism, Cryptococcus neoformans. It commonly manifests as an opportunistic infection in patients with advanced HIV (human immunodeficiency virus) infection, haematological malignancies and patients on immunosuppressive therapy, and usually involves the central nervous system, skin and lungs. Patients initially manifesting as biliary cryptococcosis is extremely rare. There have been case reports of hepatobiliary cryptococcosis diagnosed on histopathological examination in both immunocompetent and immunocompromised patients. A rare case of biliary cryptococcosis diagnosed on cytological examination is presented.
