Mammary carcinoma with osteoclast-like giant cell is a very rare subtype of invasive ductal carcinoma not otherwise specified of the WHO classification of Breast tumors . This tumor has a similar prognosis to other breast carcinomas of identical grade and stage. Nevertheless, its clinical and radiological presentation is falsely reassuring and can lead to a damaging diagnosis delay. Conversely, using fine needle aspiration, the cytological diagnosis is attainable, especially in the context of a Rapid Assessment Breast Clinic (RABC). Thereby the present case illustrates its cytological features characterized by the presence of multinucleated giant cells that are also described in several benign or other malignant breast lesions and can therefore constitute a pitfall.
