A 64-year-old Caucasian woman was referred as a result of a 3-year asymptomatic cutaneous pigmented lesion on her genital area. Physical examination revealed a well-defined, smooth, brown-black plaque, approximately 1.2 cm in maximum diameter, located on her right labium majus (Fig. 1a). Dermoscopy with immersion gel showed an irregular brownish structureless background, characterized by different shades of brown color, with a blue-gray cerebriform central area, and peripheral blue-brown elongated globules, resembling in some cases leaf-like structures (Fig. 1b).
