question if EBV infection might represent a coincidence or be associated with GGA development. In our patient, the association between GGA onset and acute EBV infection represented a serendipitous discovery. In our case, the lesions showed a marked inflammatory aspect. Instead histology showed some peculiarities, such as collagen degeneration in the deep dermis and absence of mucin. Typically in GA, deposits of mucin and collagen degeneration are observed in the superficial and mid dermis. To the best of our knowledge, the presence of leukocytoclastic vasculitis in association with GA has never been reported before. Although the pathogenetic mechanism is based on a delayed-type IV hypersensitivity reaction, the presence of leukocytoclastic vasculitis suggested an EBV-related vascular injury induced by immune-complex deposition. This finding might be a peculiar aspect of GGA induced by infectious agents. Despite the fact that more than 90% of adults are seropositive for EBV antibodies, the occurrence of GGA is very rare. On the other hand, we cannot exclude that EBV-related GGA eruptions could be under-diagnosed. Indeed, serological tests and histologic examination are not routinely performed. In order to corroborate our findings, larger series are needed. Moreover, EBV serological test should be considered in case of GGA in order to clarify this association.
