A 12-year-old Pakistani girl presented to the dermatology department with a nonhealing dark lesion on the right leg that slowly grew over 6 months. Apart from atopic dermatitis, her medical history was unremarkable, and there was no family history of skin cancer. Examination revealed a round 2.5 cm black keratotic nodule on the right shin (Figure 1). It was symmetrical and uniformly pigmented. There was no regional lymphadenopathy.
