Active in fl ammatory bowel disease-related in fl ammation triggers carcinogenesis. This can result in the development of colorectal or small bowel adenocarcinoma, intestinal lym-phoma, anal cancer and cholangiocarcinoma. 1 Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) remains the surgical option of choice for patients with pharmaco-refractory ulcerative colitis. 2 We report a case of adenocarcinoma arising in the blind end of the ileal pouch in a patient with a history of ulcerative colitis and previous orthotopic liver transplant on triple immunosuppression.
