A 72-year-old male presenting with a 45-year history of intermittent palpitations that remained sustained for 2 months was admitted to our hospital. The patient had no history of ablation, and echocardiography revealed a structurally normal heart. The 12-lead electrocardiogram (ECG) is shown in Figure 1A. An intracardiac recording of atrioventricular (AV) 2:1 conduction with alternating atrial cycle length (CL) intervals of 242 and 271 ms is shown in Figure 1B. This article is protected by copyright.
