A 67-year-old man underwent catheter ablation of an idiopathic premature ventricular contraction (PVC) with a right bundle branch block and right inferior axis QRS morphology. During the PVC, activation mapping was performed in the aortic sinus cusp and the coronary venous system, which revealed the earliest ventricular activation preceding the QRS onset by 22 msec at the left ventricular summit (LVS) where a pace map (PM1) was excellent.
