Persistent macrocheilia can be caused by a plethora of local and systemic diseases, including granulomatous disorders. We report a patient with lip enlargement due to specific infiltrates of marginal zone lymphoma (MZL). An 82-year-old Caucasian man presented with a three-months history of progressive and persistent swelling of the upper and lower lips with burning sensation, weight loss of 8 kg, and fatigue.
