Figure 1 (a) Enhanced abdominal computed tomography revealed a 7-mm dilation of main pancreatic duct (solid white arrow) and a 6-mm dilation in the Santorini duct with cystic lesions (dotted white arrow) in the pancreas. (b) Endoscopic ultrasound confirmed intraductal papillary mucinous neoplasm (solid white arrow) and no nodules in the Santorini duct (dotted white arrow). (c) Endoscopic retrograde cholangiopancreatography depicted the entire main pancreatic duct (solid white arrow). However, the Santorini duct was not delineated because of the mucin production from the tumor (dotted white arrow). Figure 2 (a) The macroscopic examination of the specimen demonstrated a 12-mm cystic lesion in the Santorini duct. (b) The microscopic findings revealed papillary proliferation of highly columnar and atypical epithelial cells, including marked cytological atypia and abnormal crowding of the epithelium.
