A strong foundation in social determinants of health (SDoH) is necessary for healthcare professionals to deliver effective value-based care to older adults and individuals with complex health and social needs. Interprofessional education (IPE) programs are well positioned to integrate SDoH with knowledge and skills for complex care, especially in the care of older adults for whom competency in interprofessional team-based care and transitions across the continuum of care is essential. We describe a student hotspotting (SH) IPE course that pairs teams of students with patients having complex health and social needs, both older adults and individuals with prior homelessness, who face substantial biopsychosocial challenges and have deficits that make them functionally similar to the geriatric population regardless of chronological age. The objectives of SH are to provide a community-based approach to develop Interprofessional Education Collaborative core competencies, learn about and address root causes of high healthcare utilization, and develop skills for building trusting relationships and navigating health and social services in community-based settings. SH addresses an existing gap in health sciences education that often fails to provide students with adequate exposure to the patchwork of complex care services that are delivered outside the hospital and clinic setting.
