A 4‐year‐old child underwent primary intracardiac repair for Tetralogy of Fallot. During surgery, a large intramyocardial conal branch was found arising from the LAD and crossing the RVOT a few inches below the annulus (Figure 1). The ventricular septal defect was closed in a regular fashion. Infundibular muscle resection alone was found adequate for the relief of RVOT obstruction. The postoperative course was uneventful.
