Orthotopic heart transplantation (OHT) remains the gold standard for the treatment of end‐stage heart failure. The number of patients who have had at least one prior sternotomy while awaiting transplantation has increased over the years reaching 50% in the last ISHLT registry report. We analysed our institutional transplant activity focusing on prior‐sternotomy setting to identify the real burden of this preoperative variable and its potential consequences.
