We read with interest the letter by Andrea Mancuso on clinical significance of portal vein thrombosis (PVT) in cirrhosis and potential utility of anticoagulation. In our prospective study, thrombocytopenia and previous decompensation of cirrhosis predicted PVT development in cirrhosis suggesting that severity of portal hypertension rather than liver failure plays a pathophysiologic role1 .
