The most significant limitation of SJTs is the lack of strong outcome measures in evaluations of their predictive validity. As high-stakes examinations they are an important way for medical schools to communicate their values and create a hidden curriculum. Unclear whether other qualities are unintentionally implicitly valued or devalued. Does assessing for "resilience" encourage tolerance for systemic failures? Nuance, complexity, and the possibility of many right answers in real life are inevitably lost. We need to promote learning, assessment, and remediation of non-cognitive abilities more meaningfully and frequently throughout medical training.
