A 15-year-old, male adolescent fell on his outstretched left hand while playing basketball and developed a hyperextension deformity of the little finger. The patient was sent to the local hospital emergency department, and X-ray revealed dislocation of the metacarpophalangeal (MP) joint of the left little finger (Fig. 1). A junior orthopedic surgeon performed a closed reduction by horizontal traction and hyperextension of the MP joint without anesthesia. A repeat X-ray indicated that the closed reduction was unsuccessful (Fig. 2). Another closed reduction by a senior orthopedic surgeon with similar manipulation under local anesthesia was attempted but proved unsuccessful again. The patient was then transferred to our institution and Xray indicated a complex metacarpophalangeal joint dislocation of the little finger (Fig. 3).
