A specialist registrar in neurosurgery who changed a patient’s surgical consent form to cover up a mistake and who failed to disclose his disciplinary problems when seeking a hospital post has been struck off the UK medical register for persistent dishonesty. In August 2015 Ibrahim Hafez attempted to perform a burr hole evacuation of a subdural haematoma but inserted the drain into the patient’s brain instead of into the subdural space, a medical practitioners tribunal heard. He was suspended and excluded by his employer, Imperial College Healthcare NHS Trust. Imperial told Hafez in several letters that he must notify any prospective employers of his exclusion.
