We present the case of a 68-year-old woman admitted with dyspnoea for 1 week. During the week, dyspnoea increased gradually and shock occurred. She was in good health before without hypertension. ECG showed T-wave inversion. White cells, C reactive protein and lactate were increased, while red cells and haemoglobin were in normal ranges. Troponin-T was slightly increased and brain natriuretic peptide was significantly increased. Chest X-ray and plain CT scan showed a huge calcified aortic arch aneurysm (figure 1A,B, solid arrows), pulmonary infection and bilateral pleural effusion.
