A 38-year-old male patient, farmer by occupation and a known case of diabetes mellitus (uncontrolled, non-compliant on medication), presented with features of right-sided sinus fullness, right-sided cheek swelling and headache for 10 days to an outside hospital. The cheek swelling gradually progressed to involve the right periorbital region as well. On examination, necrotic lesions were found in the oral cavity along with a large palatal perforation (figure 1). Partial ptosis was observed in the right eye but the vision was intact.
