We thank Dr Albuquerque for his thoughtful observations regarding our article.1 Acute ischemic stroke (AIS) treatment is becoming one of the most important public health issues of our time. Recent studies confirm that speed (time-to-revascularization) is the most important factor determining patient outcomes in AIS. With compelling data and reimbursement, exciting technological advances will make stroke interventions safer and predictably effective. Telemedicine will facilitate development of multidisciplinary stroke teams in a variety of clinical settings and locations. Any committed interventionist willing to undergo adequate training can potentially provide life-saving revascularization for patients with AIS.
