We commend the authors for exploring a provocative and complex topic with a strong, multilayered statistical approach. The post hoc analyses should be interpreted prudently, but it is plausible the dose makes a difference in combination sedative therapy (12). Future work should factor heterogeneity of treatment effect for age into the trial design a priori and explore the pharmacokinetics–pharmacodynamic and safety profiles of sedative and analgesic combinations so commonly used in clinical practice (13–15).
