DSM-5 remains inconsistent in this regard, diagnosing depression based on a primary symptom of irritability in those under 18 years, but transmuting this to mania in adulthood. Such inconsistency demonstrates how poorly understood irritability remains – often conflated with anxiety, impulsivity, anger and mood states themselves.
