While hepatocellular carcinoma (HCC) can be cured with liver transplantation, recurrence of HCC posttransplant has been well described and can be divided into extrahepatic manifestation, intrahepatic manifestation, and mixed. The optimal imaging modality for surveillance of HCC recurrence posttransplantation remains unclear. This report describes a case in which positron emission tomography computed tomography (PET-CT) scans aided in detecting and locating recurrent HCC extrahepatically.
